Reset admin access

Use Settings after logging in to update the admin username or password. If you are locked out after saving in-app credentials, clear the admin_username and admin_password_hash rows from settings, then update ADMIN_USERNAME, ADMIN_PASSWORD, and rotate SESSION_SECRET in the deployment environment.
